This professional administrative staff member will fill the role of a Licensed Clinical Social
Worker/Licensed Clinical Professional Counselor providing clinical behavioral health services to
the patients of SIU Center for Family Medicine-Quincy. This person will also provide behavioral
health training and consultation services to the faculty, residents and staff of the clinic in
addition to performing administrative responsibilities necessary for operational planning,
organization, implementation and evaluation of services.

Administration: 15%
A. Works as a team member to accomplish program objectives including program
planning, evaluation and dissemination of information regarding Behavioral Health
Services.
B. Assists in the development of program protocols, service development and
assessment of the effectiveness of treatment strategies.
C. Participates in activities that assess, review and evaluate new evidence-based
opportunities to enhance patient treatments. Researches information, implements
new approaches and evaluates results. Disseminates information as appropriate.
D. Serves on committees as appointed by Coordinator of Behavioral Health.
Teaching: 20%
1. Provides educational experiences to the residents and other learners of the program
including longitudinal learning, rotation-based learning and consultations.
2. Provides noon conferences for resident/staff educational experiences.
3. Provides consultation to faculty, residents and staff regarding patient behavioral
health issues.
4. Serves as an ambassador for the clinic and represents SIU-Quincy by attendance on
committees and other related events relevant to our mission.
Research: 0%
Clinical Service 65%
1. Provides clinical assessments, treatment planning, mental health services and
behavioral therapies for patients of SIU Center for Family Medicine-Quincy.
2. Provides individual, family and group therapy services in accordance with the
treatment plan.
3. Provides assessment, consultation and behavioral health interventions as
appropriate in the medical clinic.
4. Provides continuous information to the patient’s PCP and other providers as
appropriate for coordination of care.
5. Maintains the client’s information in the medical record and assures that it is current
and complete. Fulfills other record keeping requirements as necessary in a timely
manner.
6. Participate in Center Peer Review and Quality Improvement activities.
7. Provide emergency services to patients as needed including those who are identified
as having no primary care physician.

MINIMUM QUALIFICATIONS:
1. Master Degree in Social Work, Psychology, Counseling or other closely related field.
2. Licensed or License Eligible to practice Social Work or Professional Counseling in
the State of Illinois
3. Bilingual; fluent in both Spanish and English
4. Demonstrate excellent oral and written communication skills.
5. Must possess excellent organizational skills and have ability to prioritize work
efficiently.
6. Pleasant, courteous, professional and helpful manner is essential.
7. Effective interviewing and counseling skills.
8. Possess knowledge of and experience with mental and behavioral health principles,
developmental stage through the life cycle, normal and abnormal development and
psychopathology.
9. Sensitive to the needs of the underrepresented minority populations.
10. Knowledge of compliance for medical records and professional licensure.
11. Creative problem-solving ability.
12. Ability to adapt to a stressful, ever changing situation.
13. Knowledge of personal computer system; experience with Microsoft Office: Excel
and Word.
DESIRABLE QUALIFICATIONS:
1. Previous experience working effectively with physicians and other providers in a
primary healthcare setting.
2. Knowledge, skills and experience in providing effective counseling services to
children and adults.
